Is it possible to add an immersive difficulty? For example, weak-looking mobs should actually be weak, and strong-looking ones should be strong. When I play on Gothic difficulty, the game feels too easy, but on Nightmare, it feels too hard. I find it hard to believe that a rat or a skinny goblin could deal significant damage with a branch. Even bloodflies could poison you for a long time but shouldn't deal much damage. Things like that
